When deciding to undergo a breast reduction surgery in Los Angeles there are many questions and concerns that arise, one concern being: how badly will this operation hurt? Generally, once a patient has decided to go for a consultation, their lives have been dramatically affected by their breast size and they are ready to make a change. They have carefully thought about their options and have significant motivation to undergo the operation. Though the decision has been made, whenever an operation is involved, it is only natural for patients to worry about their recovery and how they will feel after.

In our experience, having performed hundreds of breast reduction surgeries throughout Los Angeles, California, we have found that a breast reduction surgery is less uncomfortable and painful than a breast augmentation surgery where implants are inserted and the breasts are made larger. A very common misconception about breast reduction surgery is that it is very painful. While there is pain associated with this operation, as with any other, most patients report experiencing some grogginess and shoulder stiffness or discomfort, but no significant pain the day after surgery. The patient will be prescribed pain medication in order to reduce any amount of discomfort felt after the operation to make recovery as easy as possible.

During a breast reduction surgery there is no modification made to the chest wall musculature like in a breast augmentation, where the pectoralis major muscle is released and lifted for an implant to be inserted under the muscle. This release and modification induces more pain than the removal of breast tissue. A breast reduction is approximately three hours long, depending on the surgeon and the amount of work that needs to be done. The main goals of a breast reduction are to perform a breast lift while reducing the volume and weight of the breasts, and to improve the symptoms associated with large, heavy breasts. The scars associated with breast reduction surgery will be a lollipop like scar: around the areola and a vertical scar straight down the breast, with a horizontal scar along the inframammary fold line. Scar formation and healing is different from person to person and is dependent on the technique of the surgeon, as well as the individual skin type and genetic factors that may increase the susceptibility to keloid scar formation.

Patients who present with a history of neck pain, upper back pain, shoulder pain, deep shoulder grooves from bra straps, and chronic rashes or infection due to sweating and skin-on-skin rubbing, are good candidates for breast reduction surgery in Los Angeles. It may be possible to get this operation covered by insurance depending on the policy coverage, as well as adequate proof that it is a medically necessary procedure. When consulting with a board certified plastic and reconstructive surgeon, all of your options will be thoroughly discussed and all questions will be answered.

Breast revision surgery can be extremely complex and the proper execution of such a procedure requires exceptional surgical technique, experience, and extreme attention to detail. The difficulty in these particular cases lies in the procedure being performed as well as the results from previous surgery/surgeries. Generally, any woman undergoing a breast augmentation will need some sort of a breast revision in the future, however, there are many other reasons that women may need to undergo a breast revision surgery. Dr. Golshani specializes in this area of plastic surgery and has extensive experience correcting some of the most difficult cases. When deciding to undergo any sort or augmentation or revision, it is extremely important for the patient to see a board certified plastic and reconstructive surgeon with significant experience and expertise in the field. Dr. Golshani has properly revised and corrected thousands of patients who have suffered from poor augmentations done by other surgeons, complications with implants, and has reconstructed thousand who have undergone bariatric and mastectomy surgeries.

The need for breast revision surgery emanate from a number of different causes ranging from problems with the surgical placement or position of the original implant, problems with the actual implant, problems with the patient’s breast tissue characteristics, and sometimes a combination. Improper surgical placement or position of an implant can create problems such as bottoming out and double-bubble deformity or high riding implants. Bottoming out occurs when the implant appears low on the chest and the nipples appear to be in an overly high position. Bottoming out often requires reconstruction of the internal capsule and often a breast lift with removal of skin along the bottom of the breast is required. Double-bubble deformity occurs when implants are improperly placed and are positioned too high. In order to fix this complication, patients generally have to undergo a capsulotomy (release of the scar tissue) as well as replacement of the implant.

Due to the fact that implants are a man-made product, there is always a chance for the implant to rupture or leak in both silicone and saline. It is generally obvious when a saline implant ruptures or leaks because the breast will look deflated, however, silicone can appear differently therefore it is important to continually follow up with your surgeon to prevent complications. If one is to arise, it should be replaced as soon as possible.

Implants that are teardrop shape or anatomical, have a possibility to rotate over time creating a distorted and abnormal appearance of the breast. Patients with this complication will have to undergo a removal and replacement of the implant and change to a round implant. Some patients who undergo a breast augmentation desire a size change, whether larger or smaller, and may also require a breast lift to accompany the second surgery.

In some cases, patients experience problem’s with their breast tissue characteristics and may require a revision or reconstructive surgery. In a small number of women who undergo breast augmentation, capsular contracture is possible to occur. This happens when scar tissue forms around the implants, making it hard to touch and visibly deformed. In order to fix these problem patients must undergo a capsulotomy and/or capsulectomy depending on the professional judgment made by the surgeon. It is very common for women to suffer from breast asymmetry, however in some cases, the difference can be severe. Women who suffer from severe breast asymmetry require breast implants that are different sizes to best create an even, symmetrical appearance.

Breast reconstructive surgery is extremely complex and patient undergoing this type of procedure need to be treated very compassionately due to circumstances that have highly affected their quality of life. Not only does Dr. Golshani do many breast revision surgeries, he is also an expert in breast reconstructive surgery. He has extensive experience in post bariatric reconstruction including extremely difficult breast lifts, reductions, and asymmetry correction. For patients undergoing such a drastic change in their body, surgery is often required to correct the effect of severe obesity and fast weight loss. In addition, we regularly see patients wanting to undergo breast reconstructive after mastectomy. Recreating breast for women who have undergone a mastectomy is very difficult and requires exceptional skills and perfection in the art. There are different techniques for recreating breasts as well as the nipple and areola. When patients are seeking to undergo reconstruction after mastectomy, it is extremely important to consult a plastic and reconstructive surgeon with extensive training and knowledge in breast reconstructive surgery. Dr. Golshani attended the prestigious Plastic and Reconstructive Surgery Residency Program at the University of California San Francisco, where he underwent intensive and exceptional training by world renowned experts. He is highly qualified and trained in both plastic and reconstructive surgery.

At Avosant Surgical Associates we pride ourselves in providing the highest quality and care to all of our patients. We thrive on helping all of our patients to regain a part of their self that was lost and to deliver the quality and results that each patient had envisioned. Macromastia is a condition diagnosed as having excessively large breasts.

Large heavy breasts can have a very disabling effect on the lives of women suffering from this condition causing chronic symptoms such as shoulder, neck and lower back pain, sores caused by excessive skin rubbing, a decrease in physical performance, shoulder bra strap grooving and many more.  Not only do these women deal with many disabling physical symptoms, many report psychosocial effects as well that can cause low self esteem, embarrassment, anti-social behavior, and in some cases body dismorphic disorder.  For any woman suffering from large heavy breasts, many simple activities performed on a daily basis can become a burden.  Luckily, reduction mammaplasty, commonly known as breast reduction, is a surgery that offers amazing benefits for women suffering from large heavy breasts.  Breast reduction surgery is a procedure done by a plastic and/or reconstructive surgeon that removes excess breast fat, tissue and skin to create more proportionate breasts for each woman’s body type as well as to alleviate many symptoms caused by large heavy breasts.

In a recent study, published in the Plastic and Reconstructive Surgery Journal, that examined the effects of breast reduction surgery on lower back pain yielded a 35% decrease in low back compressive forces after undergoing breast reduction surgery.  Participants also demonstrated a 76% reduction in self-reported disability after having breast reduction surgery.  This study also yielded a most impressive decrease in the frequency of pain and travel in patients studied before and after the breast reduction surgery.  In addition to many physical benefits, many patients report higher self confidence and increase in overall quality of life.

Breast reduction surgery not only can dramatically change a person’s life that is chronically dealing with these disabling symptoms caused by large heavy breasts but also yields one of the highest patient satisfaction rates over any other procedure done by plastic surgeons.   Many studies have been done and are currently being performed to further support and prove the medical benefits of breast reduction surgery.  It is always recommended to consult a board certified plastic and reconstructive surgeon to decide if this procedure is right for you.

Pregnancy can be one of the most magical experiences of a woman’s life, but also one of the most damaging to her body. Pregnancy normally brings a whole host of unsightly problems including:

  • Sagging or deflated breasts, especially from breast feeding
  • Stretch marks on the breasts and abdomen caused by putting on baby weight
  • Fatty deposits in the abdomen, hips and thighs that just won’t go away
  • Excess pounds you simply can’t lose after the pregnancy
  • Sagging stomach skin, even if you manage to shed the baby weight

All of these side effects of pregnancy can leave moms feeling bad about their bodies and reluctant to put on their pre-pregnancy clothes, never mind don a bikini for mommy-and-me swimming lessons!

While many moms are eager to rectify the problem with plastic surgery, some are reluctant because they fear they may have to undergo a number of different operations, putting them out of commission for long periods when they need to be working, taking care of their kids, and fulfilling all of the other responsibilities of a woman who tries to do it all.

Fortunately, the mommy-makeover provides moms with the perfect solution to the post pregnancy blues. A mommy makeover is a streamlined plastic surgery package that can provide you with fixes to all of the problems caused by the pregnancy.

What is Involved in a Mommy Makeover?

A mommy makeover is custom-tailored to each individual mother. Typically, the makeover consists of fixes to the breast and abdomen, which are the areas most affected by pregnancy. These fixes may include:

  • Liposuction in the abdomen, flanks, or thighs to remove excess fat and fatty deposits

Often, multiple procedures can be performed in conjunction with each other. For example, your breast augmentation and tummy tuck can be done at the same time. Your doctor can remove the excess skin from the abdomen, restoring the flat, sexy stomach you had pre-pregnancy, as well as implant saline or silicone breast implants, giving you the full, perky breast you have dreamt about, during the same operation. Minimal liposuction may even be able to performed at the same time, at your surgeons discretion. A full mommy makeover procedure reduces total recovery time for mothers on the go, reducing the number of operations and down-time needed for each procedure.

Making the Choice

If you are considering a mommy makeover, the first step is receiving a consultation with Dr. Daniel Golshani. During this consult, Dr. Golshani will discuss your post-operative goals, determine what procedures, both surgical and non-surgical, are available to achieve those goals, perform a thorough physical examination, answer any questions you may have, and thoroughly discuss the risks and potential complications.

Breast Reduction Dr. Daniel GolshaniThe decision to undergo breast reduction surgery is often a life-changing event that can result in significant happiness, personal satisfaction and improvement in one’s well being. Countless women with large and heavy breasts are diagnosed with a medical condition known as “macromastia“. Symptoms include long-term neck pain, back pain, shoulder pain, grooving of the shoulders as a result of pressure from bra straps which can even result in scarring and bleeding, as well as dermatological problems such as skin maceration, infection and rashes between and underneath the breasts.

Many women also suffer from psychological symptoms including depression and poor body image. Generalized discomfort and inability to perform physical activities can also result from having very large breasts. A board certified plastic surgeon can be instrumental in providing relief of these symptoms by performing surgery which is best described as a breast lift in which the breasts are brought back to a more shapely and aesthetically pleasing position while eliminating the excess weight of each breast.

Breast reduction surgery, or reduction mammaplasty, is well accepted as a medical treatment for macromastia. It has been performed very successfully by Dr. Golshani countless times and has resulted in almost immediate satisfaction in the majority of his patients. If you are suffering from the above-described symptoms and are considering undergoing breast reduction surgery in Beverly Hills, this website can be instrumental in answering your questions with regards to options, the various techniques which are available for surgical treatment, potential risks and complications associated with this plastic surgery procedure, and alternative nonsurgical methods of management.

Our aim is to provide you with the most recent and up-to-date information as well as testimonials from patients who have undergone this breast enhancement procedure. Out-of-town patients are also welcome to contact us so that we may arrange a consultation to coincide with the time of your visit to the Los Angeles area.

Breast augmentation is one of the most common cosmetic surgeries in the world, and advancements are being made in the field each year. In addition to new surgical techniques, new implant devices are coming out as well. One of the most popular new options for women interested in breast augmentation surgery are silicone gel implants, and there are several reasons why they’re worth considering. Below are the Top 7:

1. Silicone gel implants are safe. Silicone implants, which are also called cohesive gel implants, have been used since the early 1960s, but were banned in 1992 due to concerns about the possibility of silicone leaking into the body. Since then, manufacturers have made drastic improvements to the materials that are found in silicone breast implants today.

2. The FDA also says that silicone gel implants are safe. You don’t just have to take our word for it! Study after study has shown that the new version of silicone implants is safe. Daniel Schultz, M.D., Director, Center for Devices and Radiological Health, FDA, had this to say in the press release sent out by the FDA: “The FDA has reviewed an extensive amount of data from clinical trials of women studied for up to four years, as well as a wealth of other information to determine the benefits and risks of these products. The extensive body of scientific evidence provides reasonable assurance of the benefits.of these devices.”

3. There’s a big difference between old silicone implants and new silicone gel implants. The biggest difference between the two is that silicone gel implants won’t leak. This is because the silicone strands in the newer models are cross-linked, creating a gel that is so thick that it won’t leak, even when the implant is cut. Some German studies even go so far as to suggest that the environment required for leakage isn’t possible to create in a living human.

4. New cohesive silicone gel implants look better than their saline counterparts. Saline implants wrinkle and fold much easier than silicone implants, which can have affect the way the actual breasts look and feel. Silicone gel implants, on the other hand, look and feel more “natural” than anything else available on the market.

5. Silicone gel implants feel like the real thing. There’s a reason silicone gel implants, are referred to as “gummy bear implants” – it’s because the material inside them has a similar consistency. These breast implants are form stable, which means that they’ll always hold their shape. No matter how many times the implants are handled or squeezed, they’ll always immediately revert back to their original shape.

6. The satisfaction rate for silicone gel implants is really high. Some manufacturers of saline implants report re-operation rates as high as 40 percent. The only advantage saline has over silicone is size: anything larger than a D cup doesn’t look natural when the breast implants are filled with silicone gel.

7. Saline breast implants have a silicone casing. Even saline implants are encased in silicone, which is what comes into contact with the rest of the body. No matter which type of breast implants consumers choose to go with, there is no way to go 100 percent silicone-free.

1. Is your surgeon board certified? If you are considering a breast augmentation, it is extremely important for your plastic surgeon to be board certified by the American Board of Plastic Surgery. To be board certified, the surgeon must complete training in surgery as well as a formal residency in plastic surgery, followed by passing a rigorous written and oral examination. For a patient, this guarantees that the surgeon chosen has met a standard of training, education, and knowledge in plastic surgery outlined by the American Board of Plastic Surgery. Any doctor that has completed medical school and has M.D. after their name can call themselves a cosmetic surgeon; this is NOT the same thing as a board certified plastic surgeon.

2. Do your research! When considering any type of surgical procedure, such as a breast implants, it is imperative to do your research. Find the best plastic surgeons in your area specializing in breast implants. Look at the website of the surgeon and view the before and after photos of the breast augmentations. Ask questions when you have a consultation, it is also good to see a couple different surgeons and pick the one you are most comfortable with. There are great websites that can help you find a board-certified surgeon in your area such as www.asps.org. Know the facts and be well-informed!

3. Placement of the Implant. When getting breast implants, you have an option to have the implants placed above the pectoralis muscle (subglandular) or under the pectoralis muscle (submuscular). The decision to go above or below the muscle should be left up to you and the surgeon and is generally dependent on your anatomy and personal preference.

4. Size. Breast implant sizes are measure in cubic centimeters (cc’s) and range from 120cc-850cc. When considering the size of you breast implant, forget about bra size and think in cc’s. Consider this conversion: 1cc=1mL and 30cc=1ounce. The size of breast implants will be different for each patient and will depend on your personal anatomy, original breast size and shape. In the consultation, you will have the opportunity to try on different sizes of breast implants. It is important to note that once placed inside, the will be a bit smaller in appearance. When choosing the size, consideration must be paid to your expectations after surgery and to your body type and proportion.

5. Silicone or saline. When undergoing a breast augmentation, there are two choices for breast implants: silicone or saline. Both are approved by the FDA and have three projection options: moderate, moderate plus, and high profile. Silicone breast implants are filled with a cohesive silicone gel that feels and looks more natural. Silicone breast implants have a slightly larger incision size because they come filled to a particular volume. Saline breast implants are filled with saline solution (salt water) and has a slightly firmer feel. There is a higher chance for rippling and deflation with saline breast implants. The incision size is smaller because the implant is placed in the body and is filled once implanted. During surgery, the surgeon is able to adjust the fill volume of the implant.

6. Silicone Is Safe! In 1992, the FDA put a ban on silicone breast implants due to claims that they had made patients ill. In 1997, the US Government began one of the most extensive research studies in medical history. The Institute of Medicine of the National Academy of Science (IOM) found that “Evidence suggests diseases or conditions such as connective tissue diseases, cancer, neurological diseases or other systemic complaints or conditions are no more common in women with breast implants than in women without implants.” In 2006, the FDA approved silicone breast implants by two manufacturers, Mentor and Allergan, and were placed back on the market with restrictions.

7. Incision site. There are four options for breast augmentation incision sites: peri-areolar (around the areola or nipple), inframammary fold line (under the breast), trans-axillary (in the arm pit), and trans-umbilical (through the belly button). It is very important to discuss the options with your board certified plastic surgeon and ask their recommendation for incision placement. Some surgeons do not perform all of the incisions for various reasons.

8. You can breastfeed! After a breast enhancement most women are able to breastfeed, especially when the breast implant is placed under the pectoralis muscle. Breast augmentation is a surgical procedure and carries risks and potential complications with it, it is important to consult with a board certified plastic surgeon before getting breast implants.

9. There is no guarantee. There is no guarantee that after your breast augmentation procedure that you get what you expected. It is important to have realistic expectations, consult with a board certified plastic surgeon, and do your research!

10. Risks and complications. Breast augmentations carry risks and complications. It is a surgical procedure and there is always a possibility that something can go wrong. Also, breast implants are a man-made devise and may rupture or leak. Many women require a second breast augmentation or breast revision at some point. A consultation with a board certified plastic surgeon will explain all of the potential risks and complications with surgery and breast implants.

20 Jul, 2010  |  Written by admin  |  under breasts, health

A recent retrospective study performed at Georgetown University looked at a series of 84 patients who underwent breast reduction surgery for large breasts between 2003 and 2008. Traditional indications for this surgery have been back pain, neck pain, shoulder grooving, and breast skin rashes. The authors found that almost 2 out of 3 patients with large breasts also suffered from chronic headaches and migraines before breast reduction. Of these patients more than half (53%) experienced significant relief from their headaches after breast reduction surgery. Also significantly, 90% reported an improvement in their quality of life.

Based on my clinical experience, breast reduction surgery helps to significantly improve the classical symptoms of neck pain, back pain, and poor posture. Other studies have also shown it to be helpful in improving respiratory function. I have personally witnessed it improve many lives. Whether this procedure can help reduce headahces, especially occipital headaches (originating in the back of the head) is a question that will require further study using prospective randomized trials.

At a time when we must be careful to cut healthcare costs, we must not forget that some things – such as living a new life free of headaches, neck pain, and migraines is priceless.
